During the summer of 2026 the Transportation Department moved away from our old paper/PDF form.   That system required the child’s family to:  download a form, write on it, and then upload it back onto their device to send it to the Transportaton Department.

The new forms are digital and can be filled out directly from ParentSquare.    Because families will have ParentSquare account, some of the information we require (child’s name, home address, grade level, etc) will be pre-loaded into ParentSquare.   We hope this new system is much more convenient for families.   It will also prevent any errors from the paper based form not arriving to the correct people in the Transportation Department.   

There are three main forms families may need to interact with.

TRANSPORTATION REQUEST FORM    (This form is for the standard AM and PM pick up times for the entrie school year).  

PRIVATE AND PAROCHIAL TRANSPORTATION REQUEST FORM    (This form is for the standard AM and PM pick up times for the entrie school year).  

CRPS/FRES DAILY BUS PASS REQUEST  (This form is to change bus arrangements on an as needed basis for Emergencies only.)
